    Has anyone else received something like this from an advisor for your donor?  I'm not comfortable signing something that assigns a value to the securities, especially sent we sent a standard stock receipt at the time of the gift.  I will be reaching out to the advisor but wanted to see if others had encountered something similar before I do.

    In fact, gifts of securities are property donations. As such, per the IRS, you should provide only the name of the stock and the number of shares.

    You might provide a disclaimer statement as I have offered in the past where you mention what you valued the stock at for internal purposes only. But nothing more.

    I would not provide any further detail to the advisor and share IRS Publication 1771 which references providing only a description.
