    Hello!

    We have a donor who would like to donate lab equipment to our University. We would like to provide the donor with a release of liability for the equipment. Has anyone in the community provided a release of liability for a gift in kind, and would you be willing to share the document with us to take to our legal team?

    You probably don't need a special form. Ask Counsel for a simple statement to be included in the gift agreement. Here's sample language:

    "The undersigned hereby assumes all risk of injury or harm as a result of the activities and equipment specified above and agrees to release, indemnify, defend, and forever discharge the donor from all liability, claims, demands, damages, costs, expenses, and causes of action due to death, injury, loss, or damage to the undersigned."

    But I am not a lawyer. You need to get this language from yours!
