Like many institutions we have a pretty comprehensive naming policy in place. What we don't seem to have is the forms that might follow such a policy. Such as: A form for the proper wording of the plaque (mostly for Advancement use) or a form that facilities might need to have completed for determining the proper positioning of a plaque, the property sign offs, etc. Does anyone have these types of forms (or others I might be missing) that would be willing to share?
